Afghan donors offer $16B in development aid

TOKYO (AP) ? International donors offered $16 billion in development aid for Afghanistan on Sunday to show there will not be a mass exodus from the country after most foreign troops pull out in two years. They stressed the aid will be closely monitored to assure it is not squandered through corruption or mismanagement.

Donors from about 70 countries and organizations, at a one-day conference in Tokyo, set a baseline for aid in the crucial period through and beyond 2014, when most NATO-led foreign combat troops will leave and the country will assume responsibility for most of its own security.

Afghan President Hamid Karzai said the country faces a hard road ahead, but vowed to improve security and fight corruption as it moves toward a more self-reliant future.

The $16 billion through 2015 is near what the World Bank believes Afghanistan needs to close the gap between how much money it can afford and how much it needs to sustain its transition. The Japanese hosts had said before the conference that they hoped to get pledges of nearly $4 billion per year, so the result was roughly what they had expected.

A follow-up meeting is to be held in Britain in 2014.

"I am encouraged that the member states are willing to mobilize $16 billion," said U.N. Secretary-General Ban Ki-moon. "Afghanistan has made important progress, but the gains are fragile."

U.S. Secretary of State Hillary Rodham Clinton said the United States, which is by far the largest donor, plans to keep its assistance through 2017 near the average amount it has been over the last decade, which would likely be under $2 billion a year.

Japan, the second-largest donor, says it will provide up to $3 billion through 2016. Germany has announced it will keep its contribution to rebuilding and development at its current level of $536 million a year, at least until 2016. The Asian Development Bank announced it is providing $1.2 billion through 2016.

Clinton, who arrived in Tokyo a day earlier from Kabul, said Afghan security "cannot only be measured by the absence of war."

"It has to be measured by whether people have jobs and economic opportunity; whether they believe the government is meeting their needs," she said.

But the donors at the gathering also expressed strong concerns over how the money will be handled. The meeting in Britain in 2014 will check progress toward "mutual accountability" and a review and monitoring process to assure that development aid is not diverted by corrupt officials or mismanaged ? both of which have been major hurdles in putting aid projects into practice thus far.

Afghanistan, one of the world's 10 poorest countries, has received nearly $60 billion in civilian aid since 2002. The World Bank says foreign aid makes up nearly the equivalent of the country's gross domestic product.

The donors in Tokyo stressed that the aid has made a difference.

In the decade since the U.S. invasion in 2001, there have been major improvements in education and health care, with nearly 8 million children, including 3 million girls, enrolled in schools. That compares with 1 million children more than a decade ago, when girls were banned from school under the Taliban.

Improved health facilities have halved child mortality and expanded basic health services to nearly 60 percent of the population of more than 25 million, compared to less than 10 percent in 2001.

But the flow of aid is expected to diminish after international troops withdraw, despite the ongoing threat the country faces from the Taliban and other Islamic militants. And along with security issues, donors are wary of widespread corruption and poor project governance.

"We will fight corruption with strong resolve wherever it occurs, and ask the same of our international partners," Karzai told the donors. "Together we must stop the practices that feed corruption or undermine the legitimacy and effectiveness of national institutions."

The aid is to be tied to a mechanism that will regularly review how it is being spent, and to guarantees from Kabul that it will seriously take on its deep-rooted corruption problems ? what the conference called a roadmap of accountability. Kabul must also demonstrate efforts to improve governance and finance management, and safeguard the democratic process, rule of law and human rights ? especially those of women.

The $4 billion in annual civilian aid comes on top of $4.1 billion in yearly assistance pledged last May at a NATO conference in Chicago to fund the Afghan National Security Forces from 2015 to 2017.

The U.S. portion is expected to be in the decade-long annual range of $1 billion to this year's $2.3 billion. Officials declined to outline the future annual U.S. allotments, but the Obama administration has requested a similarly high figure for next year as it draws down American troops and hands over greater authority to Afghan forces.

The total amount of international civilian support represents a slight decline from the current annual level of around $5 billion, a number somewhat inflated by U.S. efforts to give a short-term boost to civilian reconstruction projects, mirroring President Barack Obama's decision in 2009 to ramp up military manpower in the hopes of routing the Taliban insurgency.

NASA unveils Orion capsule during ceremony

ScienceDaily (July 3, 2012) ? The Orion capsule that will make the first flight test into space was celebrated Monday morning as the cornerstone of a new era of exploration for America's space program.

The spacecraft's aluminum-alloy crew pressure module arrived at NASA's Kennedy Space Center in Florida on Friday, June 29, where it will be built up into a fully functioning spacecraft ahead of a test flight slated for 2014.

"This starts a new, exciting chapter in this nation's great space exploration story," said Lori Garver, NASA deputy administrator. "Today we are lifting our spirits to new heights."

Orion will be the most advanced spacecraft ever designed. It will provide emergency abort capability, sustain astronauts during space travel and provide safe re-entry from deep space.

The 2014 uncrewed flight, called Exploration Flight Test-1 or EFT-1, will be loaded with a wide variety of instruments to evaluate how the spacecraft behaves during launch, in space and the through the searing heat of reentry.

Later Orion spacecraft will take astronauts on missions to destinations far beyond Earth, such as to an asteroid and Mars.

"Ladies and gentlemen, we're going to Mars," proclaimed U.S. Sen. Bill Nelson, D-Fla., who joined Garver and other officials to welcome the Orion spacecraft. "We know the Orion capsule is a critical part of the system that's going to take us there."

Designed with astronauts in mind, Orion will take crews beyond low-Earth orbit for the first time since 1972, when Apollo 17 completed the last moon landing. The Space Launch System, or SLS -- a gigantic rocket akin to the Saturn V that launched the Apollo spacecraft -- is being developed to launch future Orion missions to deep space. The first launch of the SLS, with Orion atop, is scheduled for 2017.

Astronaut Rex Walheim, who flew on the final space shuttle mission and has had a leading role in the development of Orion, said the capsule can be the principal spacecraft for 30 years of human exploration of the solar system.

"It's the first in a line of vehicles that can take us where we've never gone before," Walheim said. "It'll be a building block approach, we'll have to have a lander and a habitation module, but we can get there."

Although the design is reminiscent of the landmark Apollo capsule that took men to the moon, the interior of the spacecraft if significantly more advanced. Its guidance, navigation and life support equipment have seen significant improvements in size and capabilities.

"The systems on this spacecraft, it's bigger than Apollo and it has to stay in space longer than Apollo, so it has to be better than Apollo," said Bob Cabana, director of Kennedy and a former shuttle commander.

For now, the focus for NASA and Lockheed Martin, the spacecraft's builder, is on preparing this capsule for space in 2014. During the EFT-1 mission, a Delta IV-Heavy rocket from United Launch Alliance will lift the spacecraft into orbit. Its second stage will remain attached to the capsule and will be fired to raise the Orion's orbit to 3,600 miles, about 15 times higher than the International Space Station. The mission will last only a few hours, long enough to make two orbits before being sent plunging back into the atmosphere to test it at deep-space reentry speeds.

Assembly at Kennedy will take place in the high bay of the Operations and Checkout Building, or O&C. The O&C was refurbished extensively in 2006 and has been outfitted with large fixtures and tools to turn the aluminum shell of Orion into a functioning spacecraft complete with avionics, instrumentation and heat shield.

The space-bound Orion was welded at NASA's Michoud Assembly Facility in New Orleans, in the same factory that built the external tanks for space shuttle missions.

NASA's Ground Services Development and Operations Program, or GSDO, oversaw development of the mobile launcher that will provide a launch pad for the SLS and Orion missions. The program also refurbished Launch Pad 39B, designing a new pad structure emphasizing flexibility.

"A vehicle can come in any shape and any size and be able to launch from this pad," said Pepper Phillips, manager of the GSDO program.

The Orion program, based at Johnson Space Center in Houston, calls on multiple NASA centers to team up for the missions including EFT-1.

"Ultimately, we're going to fly as one big team," said Dave Beaman, Space Launch System spacecraft and payload integration manager based at Marshall Space Flight Center in Huntsville, Ala. "The fact that we're two separate programs, and having the GSDO program here at Kennedy, it gets some early coordination going and gives a chance to iron out some wrinkles."

Although EFT-1 will be launched aboard a Delta IV-Heavy rocket, it will use a stage adapter design that also will connect the Orion to the SLS.

"You want to make a part that can be designed for the Orion flight, as well as the SLS flights so you only have to design it once," Beaman said. "We'll get flight data on the performance, which really helps."

The spacecraft arrived at Kennedy nearly 50 years to the day that the center was born. Both occasions give NASA a great chance to set future milestones that will be as celebrated as those already achieved, Garver said.

"It's a great day and great way to celebrate 50 years of success and talk about 50 years in the future," Garver said.

Seasonal changes in outpatient antibiotic use significantly alter seasonal patterns of drug resistance

ScienceDaily (July 2, 2012) ? A new study published in Clinical Infectious Diseases and available online shows how seasonal changes in outpatient antibiotic use -- retail sales of antibiotics typically get a boost during the winter -- can significantly alter seasonal patterns of drug resistance. The findings suggest that hospital campaigns to reduce inappropriate antibiotic use should be coordinated with efforts in the broader community if they are to be most effective.

In the study, Dr. Ramanan Laxminarayan, director of the Center for Disease Dynamics, Economics and Policy in Washington, D.C., and research scholar at Princeton University, and Eili Klein and Lova Sun at Princeton University show a link between changing rates of antibiotic consumption and resistance. They also suggest that restrictions imposed by hospitals may be undermined if usage at the community level is not addressed. "Considering that approximately 260 million antibiotic prescriptions are filled each year," they noted, "individual hospitals' efforts to restrict antibiotic usage are unlikely to have a large effect on certain pathogens unless complemented by and coordinated with campaigns at the community level."

Dr. Laxminarayan and his colleagues demonstrate that highly seasonal temporal relationships exist between some combinations of prescriptions among five classes of antibiotics (representing almost three-quarters of yearly antibiotic prescriptions) and resistance levels of two bacteria, Escherichia coli and methicillin-resistant Staphylococcus aureus (MRSA). Specifically, resistant E. coli and MRSA were significantly correlated with lagged antibiotic prescriptions for drugs that were highly prescribed, but uncorrelated with antibiotics that were not used as often.

To analyze prescribing patterns, the researchers relied on data collected from U.S. retail pharmacies from 1999 to 2007. Information about resistance came from a repository of test results collected from more than 300 laboratories spread throughout the country. In nearly all cases analyzed, a one-month lag was found between high antibiotic prescription levels and the prevalence of resistant E. coli and S. aureus.

Because the sheer quantity of antibiotic consumption is still the main driver of resistance, Dr. Laxminarayan said, "decreasing inappropriate use through flu vaccinations and better education of both patients and physicians on when to use antibiotics will have an immediate impact." The United States still uses more antibiotics per capita than most comparable countries, and "there is room to lower prescribing without compromising on outcomes." The researchers plan future studies to examine other combinations of antibiotics and resistant bacteria, and to specify subpopulations of the U.S.
